At the heart of Centennial’s Services is our Client-Centered Team-Based Model of Care. This model helps us give each person the support they need in a way that fits their life. It’s all about working together, using proven tools, and making sure your care helps you stay well for the long term.

How Do We Do This Work?

Our model is based on Whole Person Care. That means we don’t just treat your symptoms, we also look at everything that affects your health and well-being. This includes things like your mental and physical health, but also important parts of your life such as housing, employment, access to food, and connection to your community. These are called social determinants of health.

Our Case Managers work with you to make sure these needs are met. We believe that when your life feels more stable, your mental health improves too.
We also provide help for people who need more urgent or serious care. This includes:

  • 24/7 crisis response (in-person or through telehealth)
  • Crisis counseling
  • Referrals to hospitals or detox programs
  • Residential respite care, which gives people a safe place to stay when they need support

If medication is part of your care, our medical team is here to help with psychiatric evaluations and prescribing medications, all while working closely with your full care team.
